Hubbert Homes specializes in CalAIM-aligned housing and care for people with complex medical, behavioral, and social needs, working closely with Medi-Cal managed care plans, hospitals, and community partners to keep members safe, housed, and connected to treatment and support

Where We Operate

  • Alameda County – including Oakland and surrounding communities


  • Santa Clara County – including Sunnyvale, with a service area that supports members referred from San José and nearby cities


Residents may come from anywhere in California, but referrals typically connect to us through Medi-Cal managed care plans, hospitals, health systems, jails/prisons, treatment programs, and community-based organizations in these counties.

  • No Deposit Necessary: We offer all amenities without requiring an initial deposit, making it easier for individuals to access the help they need.
  • Stability and Support: From day one, we provide essential resources and stability, helping residents on their path to permanent housing.
  • Community Connection: We foster a supportive environment through weekly meetings and co-support activities, helping residents build lasting connections.
  • Access to Resources: We partner with local food banks and other organizations to ensure residents have access to essential services and support.
  • Support During Recovery: We perform weekly UA/BA testing to help maintain a sober and safe living environment.
