Short-term, 24/7 supervised housing for people who are too sick for the street or shelter, but not sick enough for the hospital.
What We Provide
- A bed in a sober, structured environment
- Basic nursing oversight or health monitoring (as contracted)
- Medication support / reminders
- Meals, hygiene support, and transportation to follow-up care
- Coordination with the hospital team and ECM lead care managers
Who We Serve
- Homeless adult discharged after
surgery or acute illness
- Member completing
detox or residential SUD treatment needing a safe step-down
- Person leaving
psychiatric hospitalization with ongoing stabilization needs
