Day Habilitation Programs
Helping people build independent living skills
Structured daytime support that builds the
life skills, social skills, and routines members need to live successfully in the community.
What We Do
Day habilitation at Hubbert Homes is tailored for adults with substance use, mental health, reentry, or long-term homelessness histories. Services may include:
- Daily living skills: cooking, cleaning, laundry, budgeting
- Community skills: using public transit, navigating services, going to appointments
- Social skills: conflict resolution, communication, building supportive networks
Typical Participants
- Residents in
short-term post-hospitalization housing
- Members moving into independent units who still need day support
- ECM members whose main gaps are
skills, structure, and community
