Short-Term Post-Hospitalization Housing

Providing short-term transitional housing for adults

Time-limited housing (up to six months when authorized) for medically and/or behaviorally complex adults with no safe place to heal after discharge.

What We Provide

  • Sober, structured environment with house rules and UA/BA testing

  • Connection to outpatient care, MAT, therapy, and peer support

  • On-site groups / meetings focused on recovery, reentry, and life skills

  • Warm hand-off to permanent housing via housing navigation and ECM

Who We Serve

  • Adults leaving hospitals, rehab, psych units, SNFs, jails/prisons, or recuperative care

  • People with SUD, mental health conditions, or complex medical needs

  • Individuals experiencing homelessness or unstable housing
