High-touch care management for Medi-Cal members with complex medical, behavioral, and social needs, focused on housing stability and reduced avoidable hospital use.


ECM provides a single lead care manager who coordinates all of a member’s care—physical health, mental health, SUD treatment, dental, and social services like food and housing.


Hubbert Homes can partner with managed care plans as an ECM provider or housing-focused care team, especially for members whose primary need is stabilizing housing and recovery.
