Intensive housing navigation for members who are homeless or unstably housed, helping them move from shelters, streets, or institutions into safe, long-term housing.
What We Do
Our team works side-by-side with members to:
- Complete housing assessments & applications
- Gather IDs, benefits verification, and documentation
- Identify units or rooms in Alameda and Santa Clara counties
- Coordinate with landlords, property managers, and shelter/hospital staff
