Who We Serve

Hubbert Homes serves adults in Alameda County and Santa Clara County who are working to stabilize their health, recovery, and housing.
We specialize in supporting people with
complex needs who are often falling through the cracks of traditional systems.
Our Core Populations
We primarily serve adults who are:
- Living with a
substance use disorder (including alcohol and other substances)
- Living with
mental health needs
such as depression, anxiety, bipolar disorder, PTSD, or serious mental illness
- Exiting jail, prison, or other justice settings and needing a safe, structured place to land (reentry)
- Experiencing homelessness or housing instability, including staying in shelters, vehicles, places not meant for habitation, or couch-surfing
- Being discharged from hospitals, emergency departments, psychiatric units, SNFs, or residential treatment without a safe home to return to
Our homes and services are designed for people who need
more than just a bed—they need structure, accountability, and a team that understands recovery, reentry, and housing stability.

Where We Operate
We currently serve:
- Alameda County – including Oakland and surrounding communities
- Santa Clara County – including Sunnyvale, with a service area that supports members referred from San José and nearby cities
Residents may come from anywhere in California, but referrals typically connect to us through Medi-Cal managed care plans, hospitals, health systems, jails/prisons, treatment programs, and community-based organizations in these counties.

Clinical and Social Needs We Understand
Many of our residents are dealing with more than one challenge at the same time, such as:
- Chronic medical conditions combined with
homelessness or unstable housing
- Co-occurring
mental health and substance use issues
- Long histories of system involvement (child welfare, criminal legal system, or repeated hospitalizations)
- Gaps in income, benefits, or social support that make it hard to stay housed
Our programs are built to support these overlapping needs through
housing, structure, life skills, and connection to care—not just crisis response.
